What's your pet peeve?
On an episode of Castle, they actually showed a BG lift his thumb up over the back of a Glock and pull the hammer back.

On another show I recently saw a guy rack a double barrel shotgun (actually, you just heard the noise, but his hand was moving forward when they cut to him).
