If you're a regular standard applicant ($140 app fee, or $70 if senior) & born in USA- the ONLY thing you need to send DPS is the CHL-100 form. The company that takes your prints sends those directly to DPS.

I am almost sure it is as i have not even received a traffic ticket. i go to the DPS website and type in my dl and birthdate information in and it pulls up all my information. so would this website also tell me if it was suspended/revoked for some unknown reason.
