Anyway, this reminds me of when I worked in an ER, and a friend of mine at the time who was a local cop accidentally shot himself in the upper thigh while re-holstering his revolver while seated in his squad car. He'll tell you it was the dumbest thing he ever did. The bullet entered the outer part of his thigh just below the hip, and exited the inner part of his thigh, just above the knee. Of course, he got a raging wound infection because the discharge blew tiny bits of holster and pants fabric into the wound track.
The gun in question was whatever revolver was the standard duty weapon for the Pasadena California PD at the time. He described the caliber to me while he was still laying on a gurney in the ER as "a high velocity .38 Special," which presumably means a +P load. He had drawn the gun and hidden it under his left arm when the driver of another car several cars behind him at a stoplight suddenly jumped out of his car and ran forward toward Steve's driver's side window. It turned out that all the guy wanted was to ask directions (and he obviously wasn't very bright about how he carried that out). Steve told the guy to get the heck back in his vehicle, and then started to re-holster his gun when he accidentally pulled on the trigger, or snagged it on something, discharging the weapon.
No safety, and a round in all six "pipes," but that isn't what got him shot. What got him shot was not paying enough attention to what he was doing. If you are going to carry, and possibly deploy, a pistol of any kind - revolver, semi-auto, with or without a safety mechanism - you had better be darn sure you're paying attention. Otherwise, leave it home.

I have a .357 magnum revolver in my pocket as I type this. It doesn't come with a magazine. And to answer the original question, YES, when I'm carrying a semi-auto, I carry one in the chamber, cocked and locked, the way John Moses Browning designed it to be.
And I would still carry with one in the chamber even if my gun were a Block... I mean a Glock.
