What's the safest way to wear a holster?

I bought an inside-the-pant retention strap holster for my wife's Berretta Tomcat. She's right handed. Is the gun worn with the grip facing her right hand or is it turned the other way? Is the holster placed mid-back or slightly toward the right side?
