personally I don't carry in a Dr. office if I'm the patient. I would carry past that sign in most situations, however, with a Dr. you don't always know when & where you'll be touched.
I would speak with the Dr honestly (I would NOT tell them their sign is invalid) and if they really don't want you to carry in their office, I would find a different doctor.
as for the sign, I'd ignore it. but that's me, if you really feel like you need to say something, you could say "hey, I noticed you have a new sign on your door. what's up with that?"
