Clarification on active LEO obtaining a CHL

Clarification on active LEO obtaining a CHL
I have a cousin who is active LEO and wants to get his CHL. I know he is exempt from the class and qual but my question is on the form he needs to use. Is all he required to do is complete the original application CHL-78A marking it as active LEO? Any other forms needed? Does he need anyone to sign off on it?
