Excel form for LTC-8 and LTC-100

A few years ago, I uploaded a program from this forum that was set up where you could fill out multiple students' information (name, DL #, etc )and it would fill out and generate CHL-8's and CHL-100's for you. It was formatted so the finished forms looked exactly like the DPS forms.
Since the forms have now changed, is there an updated version of that program around?
