Both the OP and Choctaw posted pictures of their open tabs (see small x).....
I posted a picture of my Bookmarks Toolbar....(NOT open tabs). And......my open tabs when I have the site open, they are blank like both of yours......

An admin just needs to add a favicon to this site.

BOTH my Bookmarks Toolbar and Open Tabs show a blank rectangle. Firefox 18.0.1

Yep, and it's really easy. this page will help make it happen -
https://www.phpbb.com/kb/article/adding-a-favicon/" onclick="window.open(this.href);return false;. You just need a 32x32 pixel image.
An iOS/android bookmark image would be nice too. You could use the same image, just sized to 512x512.
For iOS, just upload the image to "
apple-touch-icon.png". If you can see it at that exact address then it will work.
For Android, you have to add the following two lines under the favicon code as shown in the article above (in the overall_header.html file).
