Is there a downloadable copy of the CHL application that I can take a look at? I've never seen one and am curious to what all it asks and what the instructions say.
Additionally, what kind of address information does it require? For more than a decade, I've been using mail boxes etc places for my primary address on everything. Good for privacy, but also great when I travel a lot to be able to have my mail kept and forwarded easily. Will I be able to use my mailing address on the app or do they require a residence address? I've seen some threads mention that it must match my drivers license, which is my mailing address.

There is no downloadable application, but you can request a paper application here: http://www.txdps.state.tx.us/forms/index.htm#chls" onclick="window.open(this.href);return false; You want the CR-80 form.
The application needs both a physical address and a mailing address, if they are different, and you need address history for the past 5 years.
