I have a question about the CHL application form, and I want to be sure I get it right before I send it in.

A little more than two years ago, I checked into my local hospital for treatment related to depression and anxiety attacks that were 100% job-related. I did it on my own hook and stayed for about two weeks, and could have left whenever I wanted. I stayed mostly to get away from the job, which I left shortly after my hospital stay.

Now, on the application form, it asks about involuntary commitment (mine wasn't) and a "psychiatric hospital" (mine was a regular hospital).

Further, I was not diagnosed with any of the conditions listed in the statutes, and, as I said, it was more than two years ago. My reading of the form indicates that I should just put down "none".

Am I right?

I would say you don't have to put this hospitalization on the application.
