Checklist Suggestions

In no particular order;
1. Missing information on;
a) fingerprint cards
b) affidavits
c) passport photo holder
d) application
2. Not having pictures signed by fingerprinter and applicant
3. Not including all documents in packet.

Please let me know if there is any particular omissions that are common to any of these items in a) through d).
Also, CHL's, if you made a mistake you don't see listed, let me know.

I'd like to add that both the fingerprint cards require signatures in different places. Make sure you have signed each card in the proper space.

These are listed in a DPS cover page, but I think it's a great idea to add it to our checklist. Can someone list them for me?

-2 fingerprint cards
-3 notarized affidavits
-1 passport photo holder w/ 2 signed passport photos
-Completed application form
-TR 100
-Personal or Cashiers Check/MO in appropriate amount.

I had read somewhere that scratch out on the forms could delay the process. I don't know if this is true or not, but maybe it's something to avoid or correct before submitting the CHL pack.

Write your application number on the top of every document. Sometimes things get separated and you want to make sure all the pieces find their way back to your package.
Also, make sure you have included 5 full years of employment history. If you were unemployed for a period of time, make a note as to why. You were looking for work, in school, disabled, trying to find yourself, whatever, just as long as you put something down. Well, something true, and believeable!

All you need to do is account for the time. DPS does not care, nor are the evaluating your reason from unemployment. Simply stating "not employed" will suffice.

Yep, yep! On the "extra" pasport photo especially. If it gets dropped on the floor (or somewhere else in the abyss down there), without the number, they may not be able to even read the two signatures - much less know which application the photo goes with.
Also, note the info. on passport photo holder must be done with a black felt tip pen only.

Wife & I sent packets on same day (01/02/2006). We didn't know about this site at that time so had only instructions from instructor to go by. He gave us two photos and I'm sure he told us to send both. For reasons I'm not sure of now, we only attached one photo to our apps. After stumbling on this forum we realized our mistake and I emailed DPS asking if I should send second photo. Got what is probably a standard responce saying the processing time is sixty days and to wait.
They never asked for second photo. I then thought we were in for a long wait. I was only half right. Wife got hers back in only 54 days and I had the long wait, 98 days. Go figure!
The only helpful suggestion I can add to this subject is for the Instructors to give this forum info. to students. They can learn a lot from the experts here while waiting and get a better understanding of the process.

As I sit here putting my application together for mailing, I thought of one thing we were always told to do when filling out security clearance paperwork. That is to not leave any entries blank. Put "n/a" or "none" as applicable. In this package, this would apply to the RESIDENCE HISTORY, EMPLOYMENT HISTORY, CRIMINAL HISTORY and TREATMENT HISTORY, if you don't have enough entries to fill out all spaces provided.

I received my renewal packet in the mail yesterday and I have a concern. I only recall seeing ONE fingerprint card in the packet. From reading above, it looks like I was supposed to get two. What is the best way to go about getting another one?
