My CHL instructor gave me the original TR100 "white" application form as well as the "yellow" carbon copy. The bottom of the forms both say to retain a copy for my records. Do I keep the yellow copy or make a xerox and send them both in with the other CHL paperwork?
Also, my instructor did NOT print his name on the affidavits where he was supposed to. He did sign and stamp with the notary seal.
I went ahead and filled in his printed name so that the forms were complete. This won't be an issue will it?

My instructor had our entire class fill in his information for him :)

As long as he signed and stamped it, you're good.

Most notary seals have the name printed on them, and that takes care of the requirement. The new affidavits don't even have a line for the printed name.

And yes, DPS get the original TR-100 (the white one). You keep the yellow copy.
