I don't think the "hammock" approach is secure. If you carry at 3-5 o'clock, just fold the waistband of yer tidy-whities over the top of your gun before you, ah, take your seat. The gun stays concealed, never leaves the holster, and has at least a small degree of retention.

Another advantage of my shoulder holster. I can use the restroom and the gun stays in the holster. The only one that is going to see it is a pervert.
As far as public restrooms go, at my age, occasionally it comes down to “any port in a storm”.

Since you are going into a stall, Isn't it possible that you will need some bathroom tissue from the roll and can temporarily conceal with a couple of folds?

The next time anyone offers to let me try their handgun at the range - I'm going to be wondering whether that gun spent any time in his "underwear hammock" while he was taking a poo. Might invest in a pair of range gloves now.

All kidding aside (and I'm usually as guilty as the next guy of that) it's a serious consideration. One we don't like to talk about in polite company, but the reality is this: If we carry 24/7, we're going to get caught "with our pants down."
OK. So I couldn't resist. The serious answer (for me) as a 24/7 CC? I usually carry IWB, and always at about 4:00 (consistent location--regardless of today's gun choice). When I have to do the public restroom thing, I fold the "tighty whities" over the top of the pistol and it's never been noticed (to my knowledge).
As an instructor, I usually spend a little time with my students discussing the "real-world" implications of carrying a gun in public. This is one of the topics I discuss -- not that there's a "right" answer, but that it's a reality you HAVE to consider.
Just like the reality of being unintentionally exposed (carry enough, long enough, and your coat will blow open, you will bend over wrong, or something) -- what are you going to say or do? Again, there's not a perfect answer, but standing there wide-eyed like a kid caught with your hand in the cookie jar is NOT the right answer.
I'll stop now, otherwise I'll go down another of my rabbit holes, but suffice it to say, even though I snicker like the rest of us at the topic, I think it's a good one to discuss once in a while--because it will happen sooner or later...and we owe it to ourselves to have a plan.
