residence history?

I have had a permanent address for over 10 years in Texas. This is the address on my DL. I'm about to take my CHL class. For the residence history is there a place for a permanent address on the form? The address on my DL is about a mile from my current residence. I have moved 5 times in the last 5 years and have lived at my permanent address off and on. I have not had any bills go to my permanent address in almost 4 years. What is the best corse of action? I have not seen the form to know. Please help

The application asks for a residence address and a mailing address. They do not have to be the same. But your driver's license and CHL both need to have your residence address on them.
