Thanks to all that sent an example. I am just in & used this for the 1st time today.
One thing I was surprised at:
No one that shared an example of their eval. form had any ?s about the range.

Here is what I came up with. Many had basically some of the same ?s. One form may be a yes or no. The other a 12345 bad to good rating. So all were very good & I might add very compareble.

Course Evaluation
Please take a few minutes to evaluate your classroom & range experience today. I want to know what I did right & where I can improve in the future. Thank you for your honest answers. You may use the back for any written responses.

On the scale below 1 is low 5 high. How do you rate your class experience?
How well prepared do you feel the instructor was? 1 2 3 4 5
How well did the instructor know the material required by the state of Texas? 1 2 3 4 5
Did you feel comfortable asking questions? 1 2 3 4 5
Were all questions answered to your satisfaction? 1 2 3 4 5
Were examples relevant & understandable? 1 2 3 4 5
Legal wording can get boring & hard to understand. Were the legal aspects of the course explained to your satisfaction? 1 2 3 4 5
Did the instructor hold your interest? 1 2 3 4 5

On the range.
How safely did you feel the range was maintained? 1 2 3 4 5
Did you understand what was expected of you on the range? 1 2 3 4 5
Were the range commands simple & easy to understand? 1 2 3 4 5
Did your instructor keep you from being nervous on the range? 1 2 3 4 5

Over all.
Did the class meet your expectations? 1 2 3 4 5

I can only assume that you deleted some of the posts in this thread.

I guess I didn't think about putting a range evaluation in my set of questions. There are a few times were we finished up at the range and they had previously done their course evaluation. But I may need to look at it for future classes. Always good to have input from other heads on this stuff.
Just remember shot placement is much more important with what you shoot than how big a bang you get with each trigger pull.

If don't know if the DPS requires it, but I want to commend all of the CHL instructors who use some kind of class evaluation at the end of the course. Instructors for many classes that I have taken don't have any kind of feedback mechanism. It is a proven method for improving the content of the class as well as the method of delivery.

Probably should have asked if it was alright w/ everyone to post their original but did not think of that.
