It reminds me of the time I took my youngest son to the golf course on a Sunday afternoon to get 9 holes in. We were hooked up with a couple of other guys. when we got to the green everyone marked their ball with coin. My son didn't realize what was going on and started picking them up as he exclaimed, "look at all the money." Fortunately the guys thought is was funny, so laughed it off and we carried on.

If you notice, when the guys sees the shooter he tries to take cover...behind a target.

In all fairness to the guy that ended up down range, there's enough shooting going on in what I assume are adjacent stages/bays that it could be hard to distinguish shots from those areas vs shots in the area he was in. If he was the previous shooter looking for a dropped mag, it's possible he didn't notice that the range was going hot. Luckily competition stages are designed so that targets can be addressed from only a narrow angle.

I first saw this video on another forum. The comments included that the guy downrange was also hard of hearing. That plus the other shots gong off in other bays probably led him to be complacent.

This stage setup needed two rso's. One downrange or at a spot where they could clearly see the back of the berm to call clear. This stage setup worked against them. Of course that is not an excuse - just a comment.

It is BASIC that an RSO needs to start at the back of a bay, and always be the last one up range being 100% certain the range is clear.
