Ok, after following the link I was given, and the link Steve just gave; I think it's somewhat less likely that the NRA is getting personally identifying information from people completing this survey. Since the survey is being run by a 3rd party, they'd have to give referrer information back to the NRA. Most likely, the survey company is just using the referrer info to screen out duplicates.
So I take back about 65% of what I said earlier, but the gist of my comments still stand. I'll edit what I wrote.

I haven't received one, and I don't think I would part with any information for that type of survey. Maybe I'm just paranoid.

I took the survey. It did not ask for any personal info, and it seemed to mainly be geared towards market research for selling the ad spaces in NRA's magazines. It asked a lot of questions about brands, prices, preferences, spending habits, etc. It looked like a pretty harmless survey to me.
