So, you run each form through your laser printer 3 times? Once for each copy - white, yellow and pink?

Why not just print on regular paper?

I print 2 copies of the CHL-100. I sign one twice and give to the student and I keep a copy. If the student wants a copy I would make it for him or he can copy it elsewhere.

I created a spreadsheet somewhat like Mr. Cotton's (Thanks Charles for all you do. :) ) Mine is not as elaborate. I input the students data, print out a CHL-100 then copy the name and d/l to a CHL-8. After all students have been added, I print 2 copies of the CHL-8.

Sounds like the CHL-8 can be transmitted electronically. Is there a special way to do that? Just mail it to chlinstructor@dps?

I have not tried to create a scan of my signature - yet. I'll have to think about printing my signature too.

To answer your first question, yes..all three go through the printer (pink, yellow, white) printing the basic class information on them (date, locations, addresses, hours per topic, instructor name, instructor number if you elect, SA/NSA etc.). The beauty comes from the special chemical used to coat the NCR paper. Anything your write on the top copy (white) is transferred to the yellow and pink copies automatically. You sign once and the student signs once. Student DL # etc. You keep the bottom copy (pink) and the student keeps the original (white) to be mailed in and the first copy (yellow) for their records.

Filling out and signing all three copies may not be an issue for a small class, but for a class of 30 students...that is 180 signatures the instructor has to do (two signatures on each of three copies x 30 students). If you elect not to pre-print your instructor number on the CHL-100 for integrity concerns, then that is 180 times you have to write that.

No, you don't have to create three copies (two will do), and if you have a copy machine there in the classroom the obvious solution would be to do one copy...pop them in the copier and run yours that your are going to keep, giving the originals out to the students. But if you plan to hand them out at the range, then the issue of how to create copies comes up. Even with two copies, for a class of 30 that is 120 signatures and possibly 120 instructor numbers, and 120 student DL #'s.

It is just another method. Not all will use it, but some find it helpful.


As for your second question regarding the CHL 8. We were instructed in April to send it in as an attachment to a "contact" form the same way you do the CHL 7 class notification using the "attach files" feature found at the bottom of the page.
