This type of ND during reholstering has come up before. I am assuming this one is like the others and is from having your finger near the trigger as it is re holstered. Your finger catches on the holster edge and the inertia of the gun going in does the rest.
I understand that the glock safety saves officer lives as too many were lost trying to remove the safety in an emergency.

It does not matter how many safeties are on a weapon if the handler is stupid.

I would argue it is a perfect example of why journalists need to use proper grammar and spelling.
Don't follow you on this?

Numerous spelling and grammatical errors in the article, uses both "sally port" and "Sallie Port" as an example. It's my business and a huge pet peeve to see garbage like that. Obvious reporter didn't even use spell check. Definitely didn't proof read.
Right there with you -- and "Sallie Port" made me grit my teeth. What are they doing, hiring illiterate 5th-graders? Used to do significant copy editing and it kills me to see such glaring errors making it into print.

Swanton said investigators believe as the Waco officer was returning his sidearm to its holster, the keys he had in his hand somehow caught on the pistol’s trigger and discharged the weapon
“We’ll address the incident with additional training if the investigation finds that’s needed, but we don’t believe there was anything inappropriate taking place,”
Really? A wad of keys in your gun hand and you see no issue?
You think they would be so non nonchalant about it, if a chl would have done that?

Proving once again, for everyone's edification, that it is too easy to get lax (or casual) about safe gun handling. You don't have to be paranoid, but you do have to be a little anal about the rules.
