I had a 300WM and had the same issue with my IOR Crusader 5.7x40-56mm that weighs a ton.
Solution:
1) Ditch the 4-screws rings and get 6-screws rings (wider)
2) Stop at a Sport Authority or Dick's sporting goods and grab a black friction wrapping tape for baseball bat.
3) put a single layer of the tape (2) on the inside surfaces of the rings
4) install the scope as usual

No further problem.

Lots of great ideas so many thanks. I just ordered the lapping kit. I want to try that first and hope it works. For what it's worth I did follow the tightening sequence to the letter. Hopefully this will solve the problem. It would be nice to get more than a handful of rounds down range without this happening again.

Try the piece of paper as a tool die man used to be will shim around .003 at a time so after lapping try it. Mike

Not that i think you didn't tighten the screws enough by hand but are you using your best guess or do you have a torque wrench you tightened them with? If your enough of a tool nut you could buy a torque screw driver. The one i have is about 130 dollars and is rated pretty highly.

I don't know about putting paper towel between the scope and rings to make up and gap, but maybe some brass shim sheet?

All of my rifles I used a picatinny rail which would stiffen the action itself. Maybe try that?
