Where do you sign Web Form?

Registered online, took class, sent in forms. Now I'm told I didn't sign Web Form. I kept copies and there is no place on the form requesting signature 
. My instuctor checked forms at end of class. Why didn't he catch that? Now I'm waiting on mails to come so I can sign, copy and return to Austin. Another registered letter!
