To Be Or Not To Be-A Medical Legal Quandry

To qualify as a preexisting condition for denial of health insurance benefits, does the disqualifying disease or injury have to be manifest in the insured at the time coverage begins?
In other words, if you had a medical problem which was present within the time frame for disqualification but it was cured on the date coverage commenced, can you be denied if it occurs after that date but within the preexisting conditions exclusion period?
I just love these merry-go-round things with insurance companies. So much case history to wade through.
