Okay instructors, help me put together a checklist of things for CHL applicants. Please give me a list of everything you can think of that a CHL should verify before sending in their application. It would also be helpful if you would identify the most common mistakes.

I'll combine all of the responses into a sticky that should be a great resource and hopefully cut down on delays people are suffering, at least to the extent they are within an applicants control.
