filling out fingerprint cards

i posted in another forum but got no response so ill try here.. i just had my prints taken electronicaly this morning. they gave me new cards but there are some empty boxes: AKA, OCA, FBI, and ORI. my question is, is it okay to leave these blank or do they have to be filled in? also where on the card should i write my app. number? thanks for any help with this!

AKA box - if you have ever been legally know by any other name, it goes in that box. Otherwise, leave it blank.
OCA, FBI, and ORI should be left blank.
Put your application number in the box thats says "applicant"
