Online application lost? Not recognized?

Greetings All,
I completed the my CHL application online in FEB 2011. I was charged $72 to my credit card, reflected on my monthly MasterCard statement. I also received at time of online application the "Concealed Handgun Licensing Online Application Receipt", which showed the $72 charge (veteran + service fee), and I received the barcode checklist (which I took to assume a good initial application).
Early the following month, MAR 2011, I submitted my fingerprints via FAST. The same month I also requested and received a copy of my "CRIMINAL HISTORY", which stated "NO CRIMINAL HISTORY AT THE TEXAS DPS AS OF 03-03-2011".
After many delays and schedule conflicts, I finally took my CHL class last month, JUL 2011. A few days later, I submitted all required documents as requested by the barcode checklist.
Today, less than two weeks after submitting my documents to DPS, I received notice from DPS that I lack the following:
1. Application fee of $70 for veteran.
2. "AUTHORIZATION FOR RELEASE OF RECORDS AFFIDAVIT" (DPS has included Form CHL-85 for me to complete and return.)
3. "KNOWLEDGE OF LAWS AND ELIGIBILITY AFFIDAVIT" (DPS has included Form CHL-86 for me to complete and return.)
4. "ORIGINAL APPLICATION" (DPS has included Form CHL-78A for me to complete and return.)
I plan to call DPS first thing Monday morning, but it appears that DPS either doesn't have, or doesn't "see", a data record of my online application nor of my record of payment.
Again, I received online (at time of application in FEB 2011 at the the DPS/CHL website) a receipt of payment and also the barcode checklist page.
Anyone heard of this? I happen to have a common first and last name, and it wouldn't be the first time I've had records mixed up.
The only thing DPS did not ask for are my fingerprints. At least it appears that FAST was successful.
