Born out of Country documents?

Good afternoon everyone.
I hope this is the right place to post my questions.
I had my CHL and let it expire in 2001. I m having to go through the process again. I do not remember what documents I sent in when I was originally issued my CHL.
I have completed my online application and have also set up my fingerprints for next week. I am taking the CHL class this weekend and once I get everything together, I wanted to get this all in the mail early next week.
I have a few questions...
BOC paperwork - I was born in the UK and became a naturalized US citizen in 1998. I have a certified copy of my UK birth certificate and I also have a US passport and my naturalization papers. What should I send in? My naturalization papers have a nice stamp on it that prohibits copying. Am I legally allowed to copy it and send them a copy.
Also, I was stupid and got 3 deferred adjudications (class A & B) One was for assault (guy hit my truck and I confronted him. It was a shoving match, no injuries. I have all the certified/notarized copies of the dispositions from the local Court house, so I am good there.
I read the law and it states that there is a 5 year (this has passed) wait until I can apply. I was also wondering if the assault would fall under the 10 year wait Penal Code 42.01? I did talk to the CHL unit last week and the lady seemed to think that I should be good to go. It was her who told me to get the certified copies.
Any light that anyone can shed on my questions is greatly appreciated.
Is there anything I may be missing? I'll be happy to answer any questions.
