I came across this form and cannot find any information about it. It says that we must either indicate classroom only, proficiency only or a certificate number. Does anyone know how to go about obtaining the certificate number it is asking for? It then says it must be sent to RSD.

Sounds like an older outdated form. They don't use certificate numbers anymore.
