I watched the video and read the comments on Calguns.net. I too think it was a case failure, but the cause is hard to guess. The fact that the shooter apparently had a problem chambering the round that preceded the Kaboom suggests that the bullet may have been into the rifling at the time of ignition. That will drive pressure through the roof. Obviously, there are other possibilities. Not only does this emphasize the need to wear GOOD eye protection, it may also point out the need to make sure you don't exceed maximum overall length when loading your own ammo.

I am not saying this was the cause of the kaboom, but that was a dirty gun - and not just because of the kaboom. The OP said something at the top of his post about how the last normal cleaning was done 2,000 rounds ago. I'm not particularly finicky about cleaning my guns (although I should be), but 2,000 rounds between cleanings on a direct impingement AR is about 1,500 rounds too many. As lazy as I am, I probably clean my ARs about every 200-300 rounds.

Even after the second failure and the round that explodes was cycled into the receiver, it appears the shooter was doing something to the rifle to make it go into battery and fire.

Just watched three other videos on You Tube after searching for "exploding ar15". The similarities on actions taken by the shooter are striking. Squib loads suspected.

It looked like another round was fired after a squib... that or a double charge.
