I have been using Bravo Concealment for about a year now to conceal a HK VP40 with a Surefire light. I really like it as it fits great and conceals well. I was a little worried that I might have to fuss with the fit as the holster is molded for the pistol with the light attached, but it was perfect out of the box. They have a level II retention model and are made in Texas.

i got one from here - http://www.kt-mech.com/ - for my G19 with TLR4 light. Fits great, secure and most of the time comfortable.

Another vote for Bravo Concealment, I've bought a number of their holsters over the years and they were one of the few company's I could find that had a holster for my 1911 Commander length with a rail. Great fit and finish (wish they still made holsters in the thinner Kydex), only downside is the cost and wait. I might have to check out one of their Level 2 holsters.

Have you checked out Safariland? I had a couple of their ALS holsters for my Glock 21 that I really liked.
