Comfort. I have tried both IWB and OWB. But it has more to do with my 'sporty' seats than anything else probably. They kinda wrap around just a bit on both sides. I can sit in any chair in my house just fine. But in the car it just digs in too much.
I'm sure you've heard "dress around the gun." In this case, it sounds like you need a new car!
I don't reposition for comfort. I reposition to be able to draw. 3 o'clock just happens to be right above the seat belt buckle where I can get to it while driving. If I don't reposition it is between me and the seat and I can't reach it without excessive movement.

I just started carrying, and i found 4:00 to be most comfortable for me, 6' and 225 lbs.. Sitting in the car is not at all uncomfortable. I am carrying a RIA 1911 Compact and the holster I have distributes the weight nicely. It is a bit further back for a good draw with the seat belt on. I am considering getting a inexpensive revolver to keep in the car door or in between the seat and console (covered of course, I have been driving concealed for a while now) when I drive. The door panel pocket in my car is basically hidden when occupied.
