CHL-100 Forms?

I've recently completed a CHL training class and was given a CHL-100 form signed by my instructor.

I've read on other CHL Class websites that after you pass the tests, the "instructor will issue two copies of Form CHL-100, which you must both sign."

Further down on the same websites when describing the application process with the state, it says to "Include the first copy of the Form 100. Do not send in the second copy."

Was I supposed to receive another signed copy? If so, what is it for? Is it for my own records, and can't I just photocopy the signed Form CHL-100 he gave me?

We are required to keep a copy. Student can copy yours to keep if you want. Make sure you send in the original.

so i guess the second copy they are referring to is the one that I don't actually possess, but my intructor kept for himself.

Allright, just wanted to make sure.

Way back in the beginning you got one to send in & another one to frame if you wanted to.
Most folks that I know just put it in the same drawer w/ their CHL badge. :smilelol5: :rolll

Some instructors may not have a copying machine readily available, so they may ask the students to sign the copy that the instructor is going to keep for his / her records.

Before the CHL-100, we used a form titled TR-100 which had copies. You'll be hard pressed to find an instructor that is still using the TR-100 and I'm not sure DPS is even accepting the TR-100 anymore.

I have my students sign the original and make a copy for my records. Then I advise them to make a copy of everything for their records.
