Question about CHL Checklist please

I filled out the online application and went to FAST for my fingerprints and a photo. The checklist is confusing.

1. Since FAST took my fingerprints and photo do I still need photos to go to DPS? If so, are these just standard passport photos?

2. What is a photo holder and where can I get one? Do I need it if FAST took my photo? Where do you sign?

Assuming I don't need the above because of FAST it looks like all I need to send is

1. Checklist (with some things unchecked because FAST did them)

2. CHL form from instructor

3 I will also send a copy of my FAST receipt so they can see I used them.

Anything else? Thanks...

I'm pretty sure you still need to send a photograph for the license. I don't think the FAST photo qualifies.

You still have to submit passport photos

When you take your class, your instructor will give you the CHL100 (I think thats what its called)
FAST Uploads the prints automatically, so do not send the receipt from them.
Send the checklist with only the things you have in the packet checked off.
