Are we supposed to start using the new one now?

I am going to use the new one at my next class. I think there is a transition period but since I fill them out on the computer I don't have a stock of old ones to use up.

Did DPS notify us that these forms are to be used as on an effective date? I'm not comfortable with this grapevine method of disseminating important information.

How many versions are on the DPS website? If there's two, that may cause confusion. But if there's only one version on the DPS website, I think the smart move is to use that one.
