Hey guys - We are updating our website and redesigning a few things, and something we are redoing is the form that a potential student fills out to register for our class online. I really like to be able to fill out the CHL-100s electronically, but I generally don't have enough information before I go to the class. I don't have access to a printer during the class so that I can print off forms after all the students have arrived. So my question is this:

What information do you ask for when a student is registering for your class via an online webpage?

Assume that everything is secure - we have all the security certificates because we process credit cards online. I am simply looking for what information you require when the student registers, and what information you would not require. We currently require:

Name
Email Address
Phone Number

I am thinking about requiring full address, DOB, DL/ID number. Or maybe I just buy a small printer that I take to class with me..... What to do, what to do......

As a consumer I would pass on providing you my DL number and full address before class.

Buy the printer. I too would be leary of providing all that information on a small businees web site, particularly after all the cc compromises that have hit big busineeses that employ IT departments have had lately.

You can get fairly inexpensive BW laser printers that are reasonably portable.

Why do you want address and DOB?

I know address can be used to verify CC pmnts, but you certainly don't need DOB. I ask for DL/ID #, but don't require it.

Iguess I am old fadhiond and (not cmputer davy .
When they get to the class (and all that call me make it) I have then fill out a release like we signed when we went to the
class in Austin. wl Dl and state license from there state. ph # or email..

What do I care when they were born or where the live or there ss#

I accept cash, checks or something of equal value of the $125.00 I charge. Tomatoes, eggs, deer meat, choclate cakes or pies.

Buy the printer. You can get small laser printers for around $100 these days, and it's a tax deduction at the end of the year.

I am using 3 part NCR paper to print the basic information on the CHL 100. I fill out the CHL 8 as they arrive and check in as I am checking their ID and taking money. Just before the class begins, we fill in the student information on the CHL 100 as well as their release. I then take them up and keep them until the end of the class.... sign the appropriate areas and pass out two of the 3 copies.

My site is secure, but we don't process credit cards or even take credit card information. Students are redirected to PayPal for payment.

I capture the name, address, phone number, email address, driver's license number and state. The data is used to automatically generate all CHL-100s and CHL-8. I close registration at 5:00pm the night before class so I can print the CHL-100s. I print about 3 extra CHL-100s without names in the unlikely event I have someone show up who hasn't registered.

Charles is right. I learned the hard way the need to have a few extra CHL-100s.
