Instructor revenue sharing

It might not be appropriate to post responses to this question publicly, but as a new instructor, I would appreciate hearing from some veterans what the typical arrangements are when one instructor asks another to assist with a class (i.e., teaching one module, or helping with the proficiency). Flat rate, dollars per head, bartering services?

Negotiable. It all depends on what you are doing for them and how much, size of class, amount taken in per student, etc.
