CR-85 and CR-87 forms replaced

Just received a box of packet materials. The CR-85 and CR-87 forms in the application material had a big X across the front of each. Loose sheets were enclosed labeled CHL-85 and CHL-87, Authorization for Release of Records Affidavit and Eligibility Affidavit. Minor changes are on each one.

The packets I received in mid November still had the old forms in them. These have a date at the bottom: 10/09.

Then I stand corrected , may have been the batch I recieved in Mid to late Nov.
I know of the last two boxes I ordered from DPS they had the new added forms in the box.
