I pre-print the forms using Acrobat Pro. I include all student ID, instructor, classroom and range data. All the students and I have to do is print pass/fail, sign and date two copies of the form. The student is told to keep a copy and send the original. I do the same, leaving a copy with the range and/or other instructor.Papacub wrote:I have a question concerning the CHL-100 form.
