On my certification of class completion form under the student portion it says student name ,drivers liscense # then it says certificate #(fail) if they dont fail do i put my certificate number there? im confused It says instructor will indicate number that is issued(where do i find that) i dont remember this section in the class.

That is no longer done. It's from the old numbered forms. It was only filled our if you failed.
