As long time quality assurance professional, I have seen this list many times. While, I have never verified the calculations, I suspect that they are pretty accurate.

Most likely errors do happen and it is more than 1%. However, errors can be detected and corrected. Thos numbers are not too wild. Just check how much money is spent on malpractice in thehealth care.
