I mailed my app yesterday. Was I supposed to include the Fast form with my application?

nope, only those items listed on the checklist

This forum is great! Everyone has already asked all the questions I have. Here I was this morning, with my small pile of papers to send out and saw the FAST sheet and was wondering... "Do I send this too?" Nope!
