DUI In Kentucky 18Yrs Ago...

So I am in the process of gathering documents for my application. I have a question.
Between 1996-1998 Not sure of the year I was arrested for DUI in Lexington Kentucky.
This was resolved by deferred adjudication and I paid a fine and sent on my way.
I know I need to report this on my application But i do not know the date when this occurred.
Kentucky has a office of records website, i paid my fee and gave all of my information. Just received
the disposition letter. There is nothing on it 100% blank. It states NO Records were found.
Since this is a misdemeanor and there are no records how do i need to list this on my CHL application?

I was a realtor in north texas for 8 years and every year we had to do an FBI background check. Nothing
ever showed on the search, this was a fingerprint and social check.

So the upshot is how do i need to handle this on my CHL application?

Send in the "official blank document" showing no records found along with your application.

But make sure you list it as an arrest on the application, but send in the document showing nothing found on your record. It won't hurt to make sure it is listed even if it has been removed from the record.
